Kate Byrnes is a senior research assistant working on the ‘Safe System Transition’ (SafeST) research study at Northumbria University. (

It aims to gain a better understanding of incident reporting in care homes and from this to co-design a systems-level response to safety issues for people transitioning between hospital and care home.
